Insurance

I am an in-network provider with Blue Shield of California, TriWest, Aetna, and United Healthcare, and can accept most Blue Cross/Blue Shield PPO plans nationwide.  I am also able to accept some other non-PPO Blue Cross/Blue Shield plans outside of California, but coverage is dependent on the specific Blue plan's eligibility for the Blue Card program.  If you live outside of California and have a Blue Plan insurance card with a suitcase on it, there is a good chance that I am able to accept your insurance.

 

If you are covered by one of these health plans, your insurance benefits may cover all, or at least most of the cost of your nutrition counseling sessions.  On average, if you do have a copay or cost share, it is often somewhere between $15-35/session.  Deductibles may or may not be applicable prior to your plan beginning payment.  Please be aware that even if you are insured by one of these health insurance plans, coverage benefits vary from policy.  It is your responsibility to verify your benefits prior to booking your first session and to understand your payment responsibilities including cost shares, copays, and deductibles.  Cost shares, copays, or deductible responsibilities will be billed to your card on file at the time of insurance billing, which is typically within about 1 week after your session.  If you have full coverage without a cost share or copay, you will not be charged anything.  If you have an HSA/FSA card, you may use that to pay your copay, cost share, deductible, or any cancellation fees.

Out-Of-Network Benefits

 

Other health insurance plans may cover your sessions if you have out-of network benefits for MNT.  Coverage for out-of network benefits varies tremendously amongst insurance plans, so please be sure to verify your coverage for out-of-network benefits for MNT with your plan prior to booking a session if you expect to have your insurance cover the cost.  You will be responsible for paying my usual and customary rate, NOT my cash pay rate, at the time of your session, and I will provide you with a receipt and SuperBill after your session, that you may then present to your insurance company for reimbursement if you have this benefit.  My usual and customary rate is $250/hour.  Please be aware that this is MORE than my cash pay rates, as my cash pay rates are discounted.  So please be sure that you understand your coverage fully before booking your session, to ensure that you will be reimbursed.  If you do not have out-of-network benefits for MNT, please sign up for your session by selecting a service from my Cash Pay Menu Here. 

 

To verify your insurance benefits, please call the number on the back of your insurance card.

Be sure to ask your insurance provider the following questions when verifying benefits:

 

• Does my plan cover nutrition counseling sessions (specifically CPT billing codes 97802 and 97803)?

• Is Brianna Meller Nutrition & Wellness or Brianna Meller (NPI 1861260382) covered under my plan? If not, what are my out-of network nutrition benefits?​​

  • Please be aware that if you have Blue Cross/Blue Shield in any state other than California, I will NOT be listed as in-network.  However, I am STILL able to accept most Blue PPO insurance (and certain other plan types) thanks to the Blue Card program!​  You'll want to specifically ask if your plan is eligible for the Blue Card program when verifying benefits.

• Do I have preventative benefits for nutrition counseling?

• Does my plan cover diagnosis code Z71.3 with CPT code 97802 or 97803?

• Is Telehealth covered?

• Do I have a copay or cost share?  How much is my copay or cost share?

• Do I have a deductible to meet before insurance pays? How much is my deductible?

• How many visits are covered?  Is there a time limit for each visit?

• Do I need a physician referral or prior authorization?

Upon booking your first session, I will ask that you put your credit card on file to secure your appointment, even if your insurance benefits cover 100% of the session rates.  If a copay or cost share is required, your card on file will be charged after our session.  In the event that your claim is denied, you will be responsible for the full cash price of the session, unless otherwise specified by your insurance plan.  Cash prices are as follows:

• $250 for the initial 85 minute consultation

• $150 for each 55 minute follow up consultation

Please note that I require at least 48 hours notice for cancellations and reschedules.  The fee for cancellations or reschedules with less than 48 hours notice is $125.  The fee for no shows is the full cash rate of the session.  Insurance will not cover the late cancellation, reschedule, or no show fee and your card on file will be charged under these circumstances.
